Every individual traveling to Hawaii with the LHS Beta Show Choir must complete this form. If 4 people are traveling, the form will need to be completed four times.
Registration Fee of $50 per traveler due immediately.
A deposit of $200 per traveler must be paid by 2/13/26.
Total trip cost must be paid by October 1st.
Due dates for airfare to be determined.
